If you have lived in Otter Tail County, submit your answers to these questions and leave a piece of history for future generations:
1. Where and when were you born?
2. Where and when have you lived anywhere else?
3. When did you first settle in Otter Tail County, and where did you live within the county?
4. Name town and county offices you may have filled.
5. If married, give the name of your spouse, and when and where married.
6. Furnish any facts worth noting, as connection with your life, and sign your name carefully.
